General information
You should include the following in your request; full name, names of parents, mother's maiden name, date of birth, place of birth, relationship to person of record, and the reason for information request.
BIRTH Certificates are filed under the person of record's last name.
Certified copies for years of birth from 1958 to the present can be ordered at most county health departments.
The fee is $17.00 each. For credit card payment they charge an extra $5.00. Additional copies are $10.00 per name requested. You can order by fax using a credit card.
No telephone requests.
If you order by mail the turnaround time is 2 weeks. No self addressed stamped envelope is required but always a good idea to include one along with your daytime telephone number.
If you go to the office it will take about 30 to 45 minutes.
No online access available.
They do have an expedited service for fax searches.
the turnaround time is next day. Additional cost is $15.75 per package. That includes $10.75 for mailing and $5.00 for use of a credit card.
Personal checks are accepted. If writing a check make it payable to Vital Records. Prepayment is required. Credit cards accepted are Mastercard, Visa, AmEx, and Discover.

For death records
Death certificates are indexed by the decedent's last name and the year of death. Include the date of birth or age at death.

The index for records from 1900 to 1939 is by groom only.
If the year is not known and the entire index must be searched, there is an additional $5.00.

Divorce Records
To send for through the mail, include the following in your request; full name, date of divorce, the county of divorce.
The state does have an index of the records and will provide a certified verification. Actual copies must be pulled from the county of issue. This office only has records from 1939 back and from 1968 to 1994. Searches must be performed at the county level for all other years. It is left up to the county to forward the records so this office will not have the record if the county did not send it to them. Records are open to the public.

Department of Motor Vehicles: Must be 16 years old to get a driver's licence. Transcripts are $2.20. You must have birthdate and drivers license number to get this. Payment must be in the form of a money order or certified check made out to the "Colorado Department of Public Safety".
